Hi,

I am trying to research OMR software which can help me do the following:

1) Create a paper form which will allow users to select certain values by ticking boxes (like a multiple choice exam form "bubble sheet")

2) Scan this form using a normal scanner (or special OMR scanner if required)

3) Output the contents of the scanned form into an easily read format (XML for example). This data can then be inserted into an existing database

I'm trying to automate an existing data entry process.

Could anyone recommend any software that they have used to solve a similar issue?

I would need flexibility in terms of creating the forms as they would change on a regular basis
