Making a sandwich. Top to toe or crust to crust?

When your making a sandwich with two slices of bread do you align the top slice exactly to the bottom slice (crust to crust) or do you go top to toe. Surely it's top to toe, I mean if you make a sandwich with one slice of bread and fold it over it's the same.
