Student Grant - Final Course Acceptance Form

There seems to be an error with the final course acceptance form.. while the letter on the front says Student Grant Scheme 2011/2012, the actual form itself says 2010/2011.. Has anybody else noticed this or do you think there would be a problem when i return the form as the date is wrong?

Hi, I can't download form - have been trying for days. Says 'error on page' everytime. Anyway around this??
