    Quick question boardies :)
    Going for my eye test tomoroow in specsavers for my provisional license application. I'm hearing something about a D.201 form? do I have to bring this form tomorrow or do they give it to me? confusing....

    When I was getting my eyes tested I brought one along, you can print it off here.

    I'm sure they'll have copies of it though it's a pretty common request.

    You mean d.502 form and you can print off this

    It's possibly too late now, but when I was going for my eye check, I printed out a D.502 form but the optician's had one (it's a common request).

    they gave me the forms i needed, actually a brilliant service in specsavers :) I met the standard to drive but On the downside I do need glasses but ahhhhh well :):) thanks guys :)

    Ah I know! but I actually do need them for studying and I have an astigmatism in my eyes!
