It seems sometimes the carrier files for iphones are not properly installed or itunes never prompts you for a carrier file update, is there anywhere that they can be downloaded or can you force itunes to install them? can the networks send them OTA?

Not sure where you could download the ipcc but if you do manage to find it, use these instructions to install it thru iTunes.

The instructions are for an old version of iTunes but it should still work

What network are you looking for ipcc for?

Yes, they can be sent OTA. Go to settings>general>about and wait there until a prompt appears to download the file. (It could take upto 5 minutes)

It only happens if there's an update to the carrier file
