You'll get a better answer in the Independent Travel forum. Lots of people there have been to them. I liked Ascusion. Ok, not a whole lot to do, but it was interesting and not over run with tourists. Lima - I lived there for 2 months. I loved the city, the water fountain park is great (as hard as it is to believe that a water fountain park could be great).

In Quito I stayed in the Secret Garden hostel. It organises great tours, which is nice if you're travelling alone. It also has group dinners at night so you can get to know people.

Lima's a massive city, how could you not find something to do. As usual, though, you need an 'in' to truly enjoy a city. I found Inka Frog hostel very helpful that way. Doing stuff depends what you're into. I'm into soaking up culture, food and music. Lima is the gastronomic capital of Peru if not South America. You must try ceviche, the national dish (fish 'chemically cooked' in lemon juice) - a wonderful and cheap 'cevichería' is Punto Azul in Miraflores.

La Paz has loads to do (from cocaine) to watching Bolívian lady wrestling. I didn't do any of those things as my priority was to get to Coroico in the tropical Yungas lowlands region two hours north of La Paz along a breathtakingly beautiful 4,970m high pass. Top relaxation spot.

Asunción I can't help you on as I bypassed Paraguay on account of not having Spanish and Paraguay being a bit boring without it.
