How do I "Officially" leave a GP Practice or do I need to?

Can anyone tell me if I can officially leave a GP practice as I am moving town?

Do I need to ask them to send my files to my new GP or what do I need to do? Write to them ?????

    Thanks in advance

What I did was to get a letter from my new GP requesting the records, give that to the old GP who then handed over the records to me personally, and then I took them to my new GP the next time I visited.

There may be other ways of doing it, but there's no official 'leaving' procedure AFAIK.

Actually, I'd half expected the old GP to ask me why I was leaving (I wasn't moving house, just dissatisfied with her lack of professionalism) but she didn't even bother asking me for my reasons.

What Alun said sounds like pretty sound advice to me OP, you are free to change your GP for whichever reason, (in your case moving town) provided you can get another one to take you on. It would be especially important that your new GP gets access your records if you have an ongoing medical condition(s), requiring medications, visits to hospital, bloods, scans etc. however, if you've only visited your old GP for something relatively small there wouldn't be much need, but you could if you so wish.
