Maintenance of Public records

I think there is too much emphasis placed on having all the
relevant paper and computer records available for members
of the public when they are using public services, i.e. Welfare, Health etc.

So what if the welfare record is not complete, it is not
the front-line staff's job to audit everyone who comes through
the door. There are just too many people to deal with.

So what if their health records a missing a few exam results, it's
the doctor's responsiblilty to take care of the patient, not the
front-line nurse or office person who has to deal with hundreds
    of people per shift.

If the beauracrats higher up the ladder were doing their job,
these kinds of problems could be measured and worked on.

Do we really need a "Big Brother" computer system that has every detail
    of you life in it?
