Statement of Illness Benefit received?

    Hi,
    I need to get a statement from the Department of Social Protection to show illness benefit payments I received last year while I was out sick from work. Does anyone know where do i get this? Can I get one online or do I have to call in to the local social welfare office?

    I needed something similar a while back,you need to write to the dept of social and family affairs (PO box 1650 d1)

    including your pps no,your request for a statement,2 line reason why,also something like:-you give the dept- person permission under the data protection act to access the records require to furnish the information requested.

    A sae speeds things up no end

    I got mine in 5 days.

    just ring the offices in Store st. they will send you out one in the post.needed one last year and thats all I had to do.
