We were a family expecting to fly out on the 9th june unfortunately our doctor advised against one of the familly travelling so we had to change the flight schedule,

we ended up paying 230 extra for a different date and we are still not sure if he can travel, should we have had to pay this fee in lieu of doctors advice not to travel

    any help appreciated

that is the reality these days with regards to booking flights. You don't use the flight you loose the money.

Travel insurance is the better option

    Yes, is the short answer.

If the person unable to travel has travel insurance, then he/she may be covered for having to cancel, but not the rest of you as you're all fine and fit to travel.

Ryanair don't care if/why you change your bookings, as long as you pay up. This is true of every airline.
