Social Welfare over Easter Holidays

Hi I'm a casual teacher and will be entitled to claim some social welfare over the Easter holidays. However, I can only find the repeat form online - there's no sign of the form which the school have to complete and stamp etc. Does anyone know where I can download this form? Thanks highly

You need to contact your sw office and they can post it out to you. You need to tell them to add you to their list of school workers to be automatically added to the mailing list for these forms to issue by the system.
