Getting doctors note for exam

Not sure if this is the right forum for this query but anyway...

Basically I've got an exam next week which I fully intend sitting, but there is a problem. Basically, this exam is my final accountancy exam. I have sat and passed my other 3 finals. However, the accountancy body I am attached to specify that you must pass all 4 finals in 4 sittings - I have always done one at a time so now only have 1 attempt left). If I fail then my other passes lapse and I am back to square one - it's an absolutely ridiculous rule and one which is unheard of in other accountancy bodies.

Anyways, it's common knowledge among students that they will grant a 5th and even 6th attempt if you can provide a valid excuse (doctors note!). I have heard of students going straight from the exam hall to the doctors just in case they may have failed.

So what should I say to a doctor once I come out of the exam?? Migraine? Fatigue?? Basically what is my best bet to get a note? Obviously it's just a precaution in case I do go south in the exam

Stress or depression would be the easiest to fake I suppose.
