I've completed the game twice on hardened and this achievement hasn't unlocked for me? Is this a glitch or what?

had a look into it apparantly its bugged . some people get it some people dont . try it on vet(have fun with that) and see if it makes a difference

I did it on Hardened on the PS3 and got no achievement either

Same thing happened to me.

Something to do with saving mid mission and returning to it after.
Try doing the campaign again except this time only take breaks between missions and not during them.
