Wiring pickups

Hey All, Have GSA 60 Ibanez and recently put a seymour Duncan Invader into it but im thinking i've a bit of problem. On the 5-way-switch i had to bridge the last two pot together to get it to work. Anyone know how to exactly to wire anything like this would be a real help. Thanks, Mark.

GSA 60 is HSS isn't it? If so this might help a bit :
