it means the player is in the pause menu most probably changing weapons

It means the player is AFK

That may be the PSN / dashboard or at the start menu

On the PC it means he is typing either in chat or into the console. Or at the menu.

Assume on the 360/PS3 it means they are in the menu or have brought up the xbox menu/PSN screen.
