Renewing Expired Provisional License

I got my provisional license in 2004, so I think it expired in 2006. I lost the provisional license when I moved house. Does anyone know the procedure for renewing a provisional license when you don't have the original copy? The only thing I can find is the letter to say I passed the theory exam and can apply for my provisional license (but that's 2004).

Will I have to take the theory test again?

I've looked online and tried ringing various numbers. Any help appreciated.

I did this a few months ago and there's two forms you fill in: An application for a replacement and a renewal form. You'll get both at the motor tax office or online.

The replacement form has to be stamped by a garda to confirm you lost it IIRC

If it's been over five years since your provisional expired you'll have to retake the test.
