Particular tax form(s) for motorbike.

    Halo to all of You guys.

    Have a question on behalf of my friend who recently bought a motorbike and needs to register it with tax. Last time when tax was paid was around 2009 meaning that bike stayed off the road for around a year (more than less). Now in order to not being charged for last year (when motorbike belonged to someone else who didn't pay tax and hasn't been using it) he needs to find a right form which has a box that allows him to put a date when he bought it and start to be charged from that specified date. There is no way anyone else know the answer for this and he need to fill it up and send ASAP.

    Going forward i need to know couple of things:

    1) What's the name of that kind of form;
    2) Where i could find such a form (online if possible)

    Any useful tricks and infos about registering motorbikes would be appreciated as well :)
