What's involved in getting it working? Jail breaking the phone? iPad? Or both?

I use a programme called Joikuspot on my E72 to tether to my iPad when out of a wifi zone, it's just like having a 3g iPad. Unfortunately it's not available for iPhone at the moment.

As above, jailbreak your iPhone and buy MyWi from cydia ($19.99). Then just connect to your iPad to that WiFi network :)

Can you use MyWi if you don't have the add-on to use tethering in your data plan?
    How's that work?

Yeah as long as you have any sort of data it will work. It basically turns your iPhone into a WiFi hotspot, so you can connect any WiFi device to it.

But didn't the providers come out and say if they caught people tethering without paying for the addon they'd cancel their service?
How likely is it that they'd notice etc?

MiWi uses the phones data package rather than an add on, I know this as I added the 7.5 Gb tethering add-on after i bought MiWi from the rock store .
After a few weeks of using it I called o2 to check my balance and was told that I hadn't used any of it. It seems O2 can't differentiate between data consumed on the phone and data consumed via MiWi.

You would be better buying a Huawei E585 modem from Three. it costs 69 euros and it is a portable wireless modem. Its cheaper than buying the 3g option of the ipad.
