Declaring car was off the road

My buddy has not driven his car since April and is going to tax it at the end of the month and get it back on the road again.

When he brings the form to the garda station to make the declaration what kind of questions will he be asked? Does he need to prove anything?

Sorry of this has been covered before.

This has come up more than once before.

The Garda merely witnesses the signature, and nothing else.
