Does Medical Card Number Change?

I am filling out a form and it asks for my medical card number. The one I currently have expires at the end of this month. I am still waiting for its replacement and was thinking of putting down the number of the one about to expire.

Will the number on the new one be different than that of the one about to expire?

Your answer to that is in your own question: "The one I currently have..."
The number on this card is your medical card number. If you're filling in a form today, and you have a medical card number today, then that's the number to use.

As to that, it probably will be the same. Essentially, you're applying to extend the validity of the current card.
If, however, your card was taken from you coz of a circumstance change such as getting a job, but you then were re-applying on a later date because you lost the job, then you would probably get a different number the next time.
