Expired EC Learners Permit

Hi all.

Got a C Learners Permit in 2008, done the test in 2008 and passed.
Applied for EC Learners Permit in Jan 2009 free of charge as I'd paid for the C learners permit.
My error was thinking this learners permit was valid for two years - it wasn't. It was only valid for two years from the start of my first C Learners Permit (July 2010)

I just noticed last night that my EC Learners permit is expired. Does anyone know what the procedure is to get another one.

Is it just a case of sending in another form with my expired EC licence attached or will I have to do the Drivers Theory Test again, along with Medical and Eyesight report??

Just sent it off with a renewal form. Everything else is still valid.

Do I need a medical report or an eyesight report??
I sent one when I got my full EC licence in August 2008 - Do I need to get another one??
Working 6am - 7pm Mon - Saturday so dunno when I'd get the time to go to the doctor.

Medical and eyesight reports are valid for 10years.
