Not sure if this is the right forum but here goes

I am out on illness benefit with work since September last year....found out i was pregnant in December....posted in maternity form to the owner/manager last week needing it back at the latest tomorrow......he still hasnt sent it back
two issues - first is it is because of him that i am out ill he was a bully and i dont want to speak to him, the company dont have a HR department and there is literally no one in there that i trust and would ring to see what is going on
second - if i dont have form back by end of tomorrow i will be too late in applying for maternity benefit :(

so my question is what do i do????

Had a similar problem before, I was working for a company and had to leave early as can not carry full term, they have come up with every excuse possible not to sign the forms, told me I would not be entitled because I was working part time (which I was not aware of as was doing the 39 hours week) told me that I've been laid off as there wasn't enough work and so on, in the end they said that the forms were completed and sent off, after giving birth I still wasn't receiving my maternity and have contacted the department only to be told that there was no application in my name, I was back checking at that time my former employer and finally 4 months later they tell me that the "forgot" to send it off. LEGALLY ONCE THE PAPERS ARE HANDED TO THEM THEY ARE OBLIGED TO PROCESS THEM, however most of the time the employer needs to send them off, so you will really need to check if that was done, if not you need to get them and make sure that the dates on it are right and your maternity payments will be back dated if the forms are late.

oh i didnt give them all the forms just the page that was relevant to them as i also sent in stamped address envelope to me so they could send it back and i could then forward on
will photocopy all the forms tomorrow and send off again to them registered post this time i think and see what happens
    thanks for that :D

If you don't want to talk to your manager I assume you have no intention of returning to work? So why not just quit? I assume they're not still paying you.

Do let me know how you get on. And don't be worrying about it, you'll get it sorted. Enjoy the rest of your time before the baby comes, there'll be plenty of sleepless nights, a lot of screaming and absolutely no time for you, but I can tell you one thing for sure, You will love every minute of it. :)
