Can self-employed write off medical expenses?

    Yes in much the same way as PAYE workers can get tax back for medical expenses.
    BTW AFAIK health insurance tax deduction is done at source and cannot be included in your yearly tax.

    Note dental is not included.

    Does it work in the same way for PAYE ?
    i.e. do I have to pay tax first then claim it back

    or can I use it as a write -off to prevent paying the tax in the first place?

    Give the details at the same time you do your self-assessment. Also, dental (at least some dental) is included - ask your dentist.
