Habitual Residence should i send?

    Im currently receiving SWA, until a decision is reached on my Disability Allowance. When i submitted my application for the DA, the officer stated that everything seems in order, and i satisfy all the requirements.

    The only thing he said he was "concerned" about with my application was the Habitual Residence. He gave me an application, which proves Habitual Residence, in case as he said... the Department might have questions about it. Told me to keep it on the side, and fill it out only if they comeback to me questioning it, but he didn't want it filled out with my application at the time of filling

    I have lived in Ireland continuously for more then two years, and have worked continuously for two years until current sickness..

    To make it clearer..When i was child my parents emigrated to the US where i lived for 15yrs.. After university studies i came to Ireland and made Ireland my new home.(I was undocumented in States) Im a (Polish citizen), my only family still lives in the States, as does US citizen sister.

    Obviously, I do not plan to return to US, because I CANT, and obviously i will not go to Poland because i do not know the country, nor speak/read/write the language

    So to my question.. I believe the main center for DA application is in Longford..Should i fill out that form and send it to them, and to my Dun Laoghaire office, even before they ask, or should i not get ahead of myself and wait for a decision first, perhaps it might be positive?
