Ownership transfer of leased land

Hi, Could anyone explain to me the implications of transferring ownership of farmland from a father to son while the land is leased out to third party? The third party is half way through a 5 year lease. Mainly I would like to know if such a transfer is possible! And if so , how does this effect the lease? Thanks, Niall
