Application is made in the District Court so contact the family law office. They used be in Dolphin Hs. (Dublin) but I'm lead to believe they have moved from there. I'm not sure where, but check the courts.ie site for contact details

have a look at Courts.ie for information on the family law section.

But if you have any remaining queries, you should speak to a solicitor, but guardianship matters (on consent) are fairly straight forward.

As ye are in full agreement about your partner getting joint guardianship, there is absolutely no need to make any application to District Court.

Can be done simply by both of ye signing the statutory declaration form in front a practicing solicitor/Peace Commissioner/Commissioner forOaths/Notary Public.

But ensure the declaration form is treated like the deeds of a house and it is kept in a safe place, as there is no official register of Guardians in Ireland.
