How to Structure a Portfolio

I'm sorry if the title made it sound as though I had an answer - all I have is a question.

I need to submit a portfolio of writing for a course I am applying to. I don't have a giant selection, I'm totaling about 20 ish pages.

Could anyone please give me advice on how to format or structure my portfolio? How should it be presented? Should I put in contact details at the back?
Should I just titled it "Portfolio of Creative Writing ___name___" or simply Portfolio ___name___?

I would appreciate any help, and hopefully this thread will help other people wondering the same.

    Inb4 "google it." I did. I wasn't satisfied.
