Medical Card: quick question

    If you are on JSA/JSB, and that is your only income, do you have to get the application form for the medical card stamped at your local welfare office?

    I read somewhere that this was the case, but there is nowhere on the application that mentions this, just says you have to get it stamped and signed by your doc..

    Anyone know? Thanks.

    If it doesn't say anything about it on the form, why would you assume the SW office would need to stamp it? When I filled out mine I gave it into the doctors office, they gave it back a few days later then I gave it back in.

    Not sure about a stamp but you have to attach documentary evidence of your income with your application.

    you have to include supporting evidence, post office slips and bank statements etc to show your level of income and mortgage details if you own your own house.

    if you are on jsb or jas,bring the form down to the dole office,get it stamped and then bing it to hse.this may take a while ask the hse for a cover note while you are waiting to be processed.

    Older application forms used to have section to be stamped by SW. More recently you just need to submit copies of your letter from SW letter awarding you JSB/JSA and recent post office receipts if you are paid by SW through post office. A section of the application form you sign authorises the HSE to make its own enquries to check with SW what you've stated in app form about your SW payments.

    Echoing Balagan's post.

    Plus double-check with HSE regarding where to return completed application form/documentation.

    Some HSE areas require first-time medical card applicants to return to their local Community Welfare Officer and not to HSE directly.
