How to get all my immunisation records?

I'm trying to get a record of what I'm immunised against to work with Nurse on Call in the Summer but I'm not sure where to start. I guess I should ring the clinic I went to as a baby, the GP I had as a child and the college doctor because they've all given me vaccines, but what about the vaccines I got in primary and secondary school? Is there a centralised record of all the vaccines I have, and if there is, how do I access it?

    Thanks!

I had to get that myself last year.
There is a database with all your immunisation history stored on it.
I cant remember the name of the organisation 100 percent.
Check out the national immunisation office at immunisation.ie.

Your local HSE HQ will have all the details. Ring them. You'll probably have to give them some details, DOB, Mother's name ,School etc. Most of these offices will have IT records going back to the early eighties.

If you trained in Ireland the ccupational Health dept, of the hospital you were in will have required all of you immunisation histoy prior to you starting (required prior to your first placement), and they give all vaccinations during trainning, so you coul ask them for a copy of the records
    Otherwise:
Your local healthcentre/clnic will have your records, the PHN from your local healthcentre will have given you your vaccinations in school. You GP will most likely have your records for your baby vaccinations and your college will have a record of anything given there. You may need a titre level for HEP B, dependig on when you last had you vaccination, but they will let you know.

    I hope this helps
