Form for revenue to stop self employment

    I'm currently getting tax forms for self employed assessment but am unemployed and have been for over a year. Can anyone tell me what form I need to let revenue know since they're not answering the phones.
    Also what form to stop the tax forms for a business name.
    Only set this all up because we were going to do some work for the OPW and they needed tax clearance but that was over 2 years ago.
