Supplementary Welfare Allowance claim form

    The Supplementary Welfare Allowance claim form is what you fill in and take to the community welfare officer to get emergency dole (ie until my JSB claim come through), correct?

    For Part 4 "Other Residents" do they really want the names/DOB/incomes of my flatmates? Or is that only referring to other people related to me? Asking my flatmates for the incomes for my dole form seems a bit awkward.

    All of the form must be filled in. If not it'll be handed back to you for full completion.

    The only income details to be provided are for yourself and a spouse/partner and children.

    You are required to produce Personal Public Service (PPS) Numbers (formerly RSI numbers) for yourself, your spouse and your children
    and evidence of any income you and your spouse and children are getting Your Child Benefit Book or Birth Certificates for any children you may be claiming for if you do not have PPSNs for them.
