Jobseekers Allowance Application Form??

Hi,

Does anybody have a copy of the Jobseekers Allowance Application Form that they can scan and post here?
Or can anyone tell what information I need to fill in?

I live a good distance from the welfare office and it's not easy to get there.
I'm signing on tomorrow and the lady who operates the hatch told me on the phone she'd allow me to fill it out and hand it in tomorrow.

Unfortunately, I ran out of credit when she was telling me what information I would need.

She said a recent bank statement and was saying something about who I live with, but that was all I got.

Can anybody help?

So I need UP6, because I've been claiming Jobseeker's Benefit for the last year, is that right?

The form is also used for applying for Jobseeker's Benefit. Do I need all that ID when apply for the Allowance?

Because your last claim was Benefit, you might need to fill in the UP1 again as it is now a different claim.
You need proof of ID every time you make a claim - you may also need to show proof of address again, you might also need bank statement from the last 6 months.

I have a bank statement.
I don't have a driver's licence and my passport was destroyed in a fire.
Will my ATM/credit card do instead? Those are on the list but hardly the same, are they?

do you have any photo id? Take everything you can cause if they don't have enough details you'll have to go back again which will delay your claim. Take birth cert & proof of address & photo id
