    Had 2nd provisional until last month.(Took test and failed it last month)..Filliing in learner permit form..Just a quick question, wondering do I need all the eye sight form for it this time...Have very good eyesight etc, just wondering if all details start from scratch and if it is required...Any advice appreciated...If this is posted on site, apologies :):) just in a rush out...

    Sorry to hear about the test, better luck next time! :)

    Nope, you don't need another test, as long as your eyesight hasn't diminished since you sent in the original one. All you need is your old licence, proof that you've sat a test, the application form, the fee and two photos.

    Confirmation letter, appointment letter, result form, certificate of failure. Any of these will do, I think. That's if the certificate of failure exists, I may have dreamed it...
