Cast in-situ concrete threshold

I attended college back at the turn of the century and while I'm aware that regulations have changed nearly entirely since then there is something I remember we thought to specify that I've never seen put into practice on sites and looking at it no it actually seems a bad idea. Cast in-situ concrete threshold under external doors.

Wouldn't this cause a possible damp penetration problem? Is there any way to detail this so that it won't cause damp penetration? Surely its also a thermal bridge and therefore should be avoided, if so why wasit ever specified?

Sorry for all the seemingly random questions, I'm just trying to get ye're views on the situation.

A good detail is to keep a board located at the inner face of the external block before pouring the floor slab.
Carry the DPM up and over this board.
Return the edge insulation around the jambs and along the face of the board.
    Then pour floor slab.

The exposed insulation and DPM can be covered by the cill piece of the door when installed.
