Creating a Html form, feeding to PHP.

    I'm looking to create a HTML form which feeds data to a PHP script that produces a custom page using css.

    I've already designed websites using html, css, and javascript (which i'll use on this form to verify).


    Does anyone have any templates i can use to work back from that have this type of setup? Or any decent tutorials that i could work my way through from start to finish.
