Declaration of Income from Insurable Employment???

Been put on a 3 day week in work, went to dole office yesterday and they gave me the UP1 Form and Declaration of Income from Insurable Employment Form- my work colleague only had to fill out UP1 Form so wondering why I need to fill out 2 forms Anyone else fill out both?
