I need to put a questionnaire on a website and would be glad if someone could give me an idea of what I should do. I have basic knowledge of html but thats as far as it goes. btw the questions will include boxes and space for people to leave comments.

I've used google docs to create a few surveys that worked very well.

Very flexible, no limit on number of responses, goes straight into a spreadsheet. Not always the simplest to use, but easy enough, and free.
