Is the medical form for applying for a provisional license only for over 70's?

    The Medical Form is only required for Learner Permits in categories C1, C, D1, D and their E-versions etc if you are under 70.

    It is required for all categories for persons aged 70 or more.

    So, assuming you are under 70 and applying for a normal car or motorcycle (Cat B or A/A1) Learner Permit then you dont need a medical form.
