Form for declaring car off the road (Tax)

Hi folks, Does anyone know where to pick this form up? Is it the Garda station, or the local car tax office? I know it needs to be signed/stamped by a Guard, just not sure where to pick is up. Oh, and does anyone know the name of it? Cheers :)

in the Garda station, its on the normal tax renewal form, its a section at the bottom of the page, and you have to get it stamped at the station.
