Recommend an online survey tool

I'm looking for an online survey tool that will allow me to customise the look & feel, and format of a survey and embed it in one of my website pages. A key requirement is that it should allow me to specify a 'return URL' to redirect to after the survey form is submitted - I don't want the survey tool writing anything back…

I need to redirect back to one of my own pages after submission.

Look at SurveyMonkey. I don't know about returning to your site - the paid version can probably do this.

If you want to have it as your own and run it fully on your own site check out http://www.limesurvey.org/ - I have used this before and while it's not perfect you can see all the source code and get something up and running quickly.
