    Why blame the 'system' when countless health service scandals occur rather than the individuals involved? Who gets the blame when social workers get it wrong? Who gets the blame when the 'experts' get it wrong?

    Does the 'in-camera' rule protect certain professionals when they get it wrong? Does the 'in-camera' rule contribute to avoiding retribution and evading responsibility?
