If a person wants a private second opinion does that person have to make a freedom of information request in order to get their own records or is the original hospital likely to give them their charts and results(either to the patient themselves or the second doctor) without the person having to go that route?

I've known of patients to look for their medical records from the hospital, for similar reasons. It's why any person writing in the notes must be careful to keep it all factual and relevant, without any personal comments.

pretty much every hospital has a Freedom of Information officer. They are probably the best people to contact regarding the process involved. You can't just wander in and ask for the notes for obvious reasons I guess

Make an official FOI request is your best option (you could make a request under Data Protection Act but same difference really).
There are specific time limits they have to reply within and there is only a minimal fee involved.
