I'm going into the 3rd Year of my 4 year undergraduate degree and Im hoping to get a grant. Have been looking at last year's form to get a feel of it -
Candidates who previously attended or are currently attending full time 3rd level / PLC courses must supply the following information in respect of each year of each course. Documenting evidence is required in respect of each course completed or part completed

Would a transcipt of 1st & 2nd year results from my university suffice? Also what would I put in the 'Qualification' section for each year?

Yep, you can get this from the Exams & Records office at your college or your admissions office.

    Hope this helps! :)
