Cavity detail around windows

    Hi,
    Just wondering what the best detail is around windows in a cavity block house. The cavity currently has 60 mm kingspan tied to the inner leaf.

    Overhead there is a 4" and a 6" lintel with dpc comeing down through.
    At the bottom there is a window cill sitting in DPC tray with 1" aeroboard at the back of it.

    At the sides there is DPC running down from the top, and the cavity is closed down to about an inch.

    Just some questions.
    1. Should there be insulation at the sides filling the inch gap and overhead between the lintels?
    2. If there should be insulation, should it sealed a bit with expanding foam to make a better job of it?
    3. How far should the dpc stick out at the sides and top?
