medical card application - what do i need?

hi all, tomorrow is my first signing at the SWO, seeing as i will be down there i think i will put in for the medical card too. does anyone know what else i need for the application? i had a medical card years ago while i was in college, will this impact the applicationin any way. thanks

The SWO does not deal with medical cards.
Go to www.citizensinformation.ie , search for medical cards and it answers your question.

You have to get the form the HSE. THere office is usually near the social welfare office. FInd out where your local office is from the Interent or the Independent directory. It will tell you what you need for the card on the form.
