    Has this gone up? I know it costs 25€ for a ten year one from what it says on the form i have but its an old form and i'm wondering if the fee has increased,i'm going up to the tax office to get it tomorrow and want to bring enough money,does anyone know?

    I don't think it changed, in fact I'm almost certain. In any case, even if it had, I think government offices usually have some flexibility to accept the old fee if someone is applying with a form that has the old fee marked on it. So just go with a cheque for €25 and you should be grand.
