Fillings and PRSI

I recently had to get two fillings and was told by the dentist that I could claim some of the cost back through PRSI. They were very vague and as my mouth was numb, I couldn’t ask too many questions!
They referred to a MED 2 form, which I have found on the net. Can anyone tell me what I need to do with this? Do I need to send it back to the dentist or straight to social welfare office? The receptionist was very unhelpful when I asked so that’s why I’m checking here.

    Thanks in advance!
