    It could be a problem with a fuel injector or it could also be a problem with a blocked fuel filter. It could be air getting into the fuel line or worst case scenario a problem with the fuel pump.

    I'd start by first of all replacing the fuel filter, at the same time add a diesel fuel additive, (directly into the fuel filter), and while your doing that, get a fuel sample and check for contamination in the fuel. If that doesn't resolve the issue to a large degree, I'd then get leak back checks done on all 4 injectors and see if there is a problem there. This is usually where the problem is on diesel engines.

    EDIT: It doesn't have a turbo, right?

    this has been going on for a couple of months so i dont think it could be fuel contamination

    Has it by any chance been happening since you changed the fuel filter last???

    I had this problem once on that type of engine, the fuel filter is at the top of the engine and the filter itself is a paper insert. The seal that fits around the plastic cover that goes over the fuel filter housing can be an absolute b*stard to get to sit back on right sometimes. Check this for a start is my advce, also, the fuel lines are clear plastic on that engine if I recall correctly so run the engine and look at the fuel lines and see if you see any air bubbles in the fuel line.

    I'd say if these checks don't reveal a problem, get leak back checks done on the injectors to see if one or more of them are acting up.

    Remove and check the EGR valve. Check carefully in the aperture after you remove it and I think you will see your problem..

    Along the lines of this very advice OP, my old man has the same type van as you and had the same problem you describe and the problem in his case was the cat was blocked with its own active material and due to this the engine couldn't breathe, I had completely forgotten about this... Like in your own case it was only effecting performance at high engine speeds...

    EGR = Exhaust gas recirculation

    its a valve that that controls the recirculation of the the exhaust gasses
