Getting MMR Vaccine Evidence for a Visa

    I need to fill out a form with a stamp from my GP saying when I had my measles, mumbs and rubella vaccine as a child ASAP. Conveniently enough he is gone for the next two days. As I live in Galway would the Western Health Board or any other authority have this on file and be able to fill out the form? Cheers

    Whoever vaccinated you will have the record, if it was your gp, you can get a copy off the secretary, if you got vaccinated through the school system then the HSE West (formerlay WHB) will have a copy of it.

    EDIT:

    I just checked Australia Etho and it doesn't appear the MMR is a requirement although TB, HIV, Hepatitis and being obese are mentioned.
