I'm not great at playing guess-the-font like some people, but you could try uploading it here -> http://www.myfonts.com/WhatTheFont/ and see if there's anything close.
Or if you just want to make a larger, cleaner version you could make an automatic vector trace of it in inkscape... or even zoom in and trace it by hand.
